The NHS Certificate of Membership is your official proof that you are an active member of National Healthcare Shield under Kaiser International Health Group.

How to Get Your Certificate

• Scan the QR code or visit the provided certificate link
• Enter your First Name, Last Name, Date of Birth, and Email Address
• Click Search

What Information Appears on the Certificate

• Group Account Number
• Group Name
• Member Name
• Member Number
• Annual Member Contribution
• Coverage Expiry Date

Downloading Your Certificate

Once verified, you can download the certificate to your phone, tablet, or computer. It is recommended to keep a copy saved for easy reference.

How to schedule Annual Physical Examination

Send an Email for APE Request for individual or group application for APE.

Remember that the APE is annual, so you are allowed exactly one APE in your 12 months of short term healthcare coverage

In your email, specify

Include the following :

1. Group name (specified in your Kaiser card)
2. Name of Hospital or Clinic and Address
3. Per member
   • Name of member (Exact name in your Kaiser card)
   • Age of member

Or if availing the APE in different hospitals and clinics, email a table

• Select APE to generate your Letter of Authorization

How to Avail Inpatient Care

Inpatient care is provided when hospital admission is required.

Step-by-Step Process

• Proceed to the Kaiser-accredited hospital
• Coordinate with the HMO Office
• The HMO Office issues admitting orders
• Complete required tests and clearances as outpatient, if applicable

During Admission

• Present your Kaiser ID and admitting orders to the admitting section
• Notify the HMO Office of your admission details
• Kaiser issues a Letter of Authorization (LOA)

Upon Discharge

• Hospital submits the statement of account to Kaiser
• Kaiser applies coverage based on your program
• Member settles any charges not covered
